Annual Check-In

Each fall, CSI chapters and regions complete the Annual Affiliation Agreement Check In. This process is required for all chapters and regions, and is designed to protect your organization, support your leaders, and confirm good standing with CSI's Affiliation Agreement.  

If any requirements are not yet in place, please note your chapter's current status and any next steps in the text box that will appear. The Chapter, Region, and National will work collaboratively to create a plan so that the chapter or region can work towards compliance.

What to expect

  • Plan to set aside approximately 30 minutes to complete the survey in one sitting. Progress cannot be saved. 
  • You'll verify state incorporation, tax exemption, insurance, and enabling documents.
  • Have copies of 
    • your enabling documents (if they've changed)
    • your financial documentation indicating financial position, including the balance of all accounts and a summary of income and expenses at the end of the last fiscal year (ending 6/30/25)
    • any non-member recruitment lists in a spreadsheet shared in compliance with the chapter’s or region’s privacy policies
  • Documents can be submitted in any of these formats: avi, bmp, doc, docx, gif, jpeg, jpg, mp3, mp4, mpeg, mpg, pdf, png, ppt, pptx, txt, wav, wmv, xls, xlsx, zip

Verifying State Incorporation

A good place to start is with the Secretary of State’s Office in the state where the chapter was incorporated. Please thoroughly research requirements as laws differ from state to state and regularly change. 

Tax exemption

Check the IRS Tax Exempt Organization database for an overview of steps that may need to be taken to remedy your status.

Insurance

The types of policies generally required by business entities are Association Directors & Officers Liability (D&O) and General Liability. Learn more about Insurance Programs.

Enabling Documents

These include the Bylaws, Articles of Incorporation or business license, and IRS determination letter of tax-exempt status. 

Ensure your governing documents and yearly processes are up-to-date and compliance with applicable laws by reviewing annually. CSI works with the law firm of Webster, Chamberlain & Bean, LLP to connect you with legal resources if needed.

Financial Documents

The documents should indicate the entity’s financial position. Examples include a summary of income and expenses and account balances. The chapter or region is not expected to use a specific template; please provide what your chapter or region uses.
